Chastity Chyld Incorporated & CDC Diabetes Prevention Program Partnership

Executive Summary

Chastity Chyld Incorporated holds the operational authority to deliver the Centers for Disease Control and Prevention (CDC) National Diabetes Prevention Program (National DPP). As fully certified health coaches within the CDC Diabetes Prevention Program framework, our leadership team is fully prepared to expand this life-saving initiative to the youth under our care.

To bridge the final gap between our current authorization and program deployment, we have applied for a United Way grant. This pending funding will specifically finance the formal PreventT2 Curriculum Training required to certify our foster youth as qualified, peer-led CDC Diabetes Prevention Lifestyle Coaches.

The Training Need & Financial Goal

While Chastity Chyld Inc. has the program authority, deployment relies entirely on secure funding to cover the professional training infrastructure for our youth.

  • The Costs: Official CDC-aligned Lifestyle Coach training generally ranges between $500 to $800 per person.

  • The Funding Role: Funding from the United Way grant application will directly cover these individual registration and certification fees.

  • The Strategic Objective: Securing these funds allows Chastity Chyld Inc. to equip foster youth with paid public health credentials, empowering them to facilitate the PreventT2 curriculum alongside the CDC.

About the PreventT2 Lifestyle Change Program

The PreventT2 curriculum is a structured, year-long, evidence-based program designed to help individuals make the lifestyle shifts necessary to avoid chronic disease.

  • Proven Clinical Outcomes: Research proves that participants with prediabetes can reduce their risk of developing type 2 diabetes by 58% (and up to 71% for individuals over 60).

  • Core Metrics: Youth coaches will guide participants toward losing 5% to 7% of their total body weight and reaching 150 minutes of physical activity per week.

  • Program Layout: A 12-month timeline split into two crucial phases:

    • Months 1–6: Minimum of 16 weekly foundational sessions.

    • Months 7–12: Minimum of 6 monthly maintenance sessions.

  • Accessibility: Youth coaches will utilize culturally adapted, CDC-approved resources available in both English and Spanish.

Program Structure for Youth Coaches

Once the United Way grant secures the required training fees, our certified foster youth will take on active leadership roles within the public health sector:

  • Cohort Management: Certified youth will lead peer support groups ranging from 10 to 25 participants for local, in-person classes.

  • Flexible Delivery: Youth coaches will be trained to facilitate the program in person, entirely online via virtual platforms, or through a hybrid approach.

  • Professional Foundation: This initiative transforms youth from program participants into certified public health advocates, opening doors for sustainable career paths.

The National Diabetes Prevention Program (National DPP)

To better address the growing problems of prediabetes and type 2 diabetes, Congress authorized the CDC to establish the National DPP, a public-private initiative that provides the framework for type 2 diabetes prevention efforts in the United States.
